We see most of our patients on an appointment basis.  If you, your child is sick or needs assistance, please call and speak to a nurse or social worker regarding your concerns.  Together, we will make a plan for your care and/or services.

If you have a scheduled appointment and cannot make it, please call us at least 24 hours in advance to cancel and we will be happy to assist you in rescheduling.

What to Bring

Please bring the following items to you/your child's first appointment:

  • Insurance card.  If you are not enrolled with an insurance plan, Medicaid or ADAP/PrEP-AP, we will work with you to apply for benefits.
  • Photo ID (school ID, driver's license, non-driver's license, or benefit card)
  • Up to date shot record, if available
  • List of medication you/your child takes


If you/your child becomes sick or injured, we can usually see you on the same day.  When the office is closed, we have doctors on call that can advise you over the phone.  Our answering service and on call doctors are always available when the office is closed.  Emergency Rooms are for serious emergencies.  Unless the problem is life threatening, please call the office first.  If we are closed, our message will give you the number for our Answering Service, (716) 608-1054, for all medical care.
